Tattoo procedures log
As a tattoo operator, you must keep a log book of all tattoo procedures.
You must enter a procedure if it’s done:
- at the premises
- for a fee.
You must use the approved form.
Keeping the log
You must have a log book for each calendar year. This still applies if you only operate for part of the year.
Each log book must be:
- in English
- on the premises at all times (for 3 years)
- easy to access.
Entering details
The staff member who does a tattoo procedure must fill out the log. This must happen at the time of the procedure.
This could be:
- you
- a contractor (on a tattoo operator licence)
- an employee (on a tattooist licence).
You must record:
- when a procedure takes place (include all dates)
- who performs it (your full name and licence number)
- how much the client paid
- what method of payment they used
- whether you issued a receipt (include the receipt number).
